Parent Consent Form (PCF)

The Parent Consent Form (PCF) must be completed annually for all students.

The PCF provides parents/guardians the opportunity to:

• acknowledge acceptable and responsible uses of student technology,
• give consent for a high school student to use social media,
• give consent for the district to use your student's photo and/or video,
• give consent for the release of student directory information and,
• give consent to participant in school clinic services, health screenings, and mental health services.
