Food and Nutrition Services Department is required to provide reasonable accommodations for students with allergies.

Parental Request Form

By completing this Parental Request Form (English or Spanish), you are acknowledging that: your child/student has a medically certified dietary need and an official FNS Dietary Order Form has accompanied this document, which should be completed and returned to the school.

Diet Order Form

Have form completed by student's physician and return to school's cafeteria manager.

What is Direct Certification- - Direct Certification is the process under which the state provides the school district with a list of students where the families are receiving assistance under the Supplemental Assistance Program (SNAP), Temporary Assistance for Needy Families (TANF), and Food Distribution Program on Indian Reservations (FDPIR). The students are eligible for free school meals, without further application, based on the information provided by the State/local agency administering those programs.
